Shall the Lake County Forest Preserve District borrow money and issue bonds to acquire and improve forest preserve lands in order to preserve and restore wildlife habitats, trail and greenway corridors, wetlands, prairies and forests; to provide flood control; and to create and improve public use areas for recreation, education and historic facilities in an amount not to exceed $55,000,000?

Waukegan Township

Shall the Illinois General Assembly enact the Bernardin Amendment which would establish health care as a basic right of every person in our state and establish May 31, 2002 as the deadline to enact a plan for universal health coverage that permits everyone to obtain decent health care on a regular basis?

Wauconda Township

Shall the Illinois General Assembly enact the Bernardin Amendment which would establish health care as a basic right of every person in our state and establish May 31, 2002 as the deadline to enact a plan for universal health coverage that permits everyone to obtain decent health care on a regular basis?

Shall the Village increase the surcharge from the current rate of up to $.75 (seventy-five cents) to a rate of up to $1.50 (one dollar and fifty cents) per month per network connection, which surcharge will be added to the monthly bill you will receive for telephone or telecommunication charges, for the purpose of improving and maintaining a 9-1-1 Emergency Telephone System?

Village of North Barrington

If a shopping mall/fashion center is built on the 108 acre parcel of land in the southeast quadrant of the intersection of Route 12 and Old McHenry Road, would you prefer that the Village of North Barrington have control over the development and receive the estimated $3,000,000 of sales tax revenue annually rather than the Village of Hawthorn Woods?

Do you approve the annexation of the 108 acre parcel of land in the southeast quadrant of the intersection of Route 12 and Old McHenry Road into the Village of North Barrington?

Shall the Village Board of the Village of North Barrington approve the development of a shopping mall/fashion center on the 108 acre parcel of land in the southeast quadrant of the intersection of Route 12 and Old McHenry Road if said parcel of land is annexed into North Barrington?

Village of Volo

Under the Property Tax Extension Limitation Law may an aggregate extension not to exceed $60,000 be made for the Corporate Fund of the Village of Volo, Lake County, Illinois, for the 1999 levy year?

Shall the annual tax rate for Corporate purposes of the Village of Volo, Lake County, Illinois, be increased from .25% to .4375% upon the value of the taxable property in the Village as equalized or assessed by the Department of Revenue?

City of Waukegan

Shall the City of Waukegan, Illinois, impose a surcharge of up to $1.50 per month per network connection on telecommunication carriers, which surcharge expense will be added to the monthly bill you receive for telephone or telecommunications charges, for the purpose of improving a 9-1-1 Emergency Telephone System?

Village of Lincolnshire

Shall the Village of Lincolnshire use the power of eminent domain to acquire the approximately 63 acre parcel at the southeast corner of Riverwoods and Everett Roads in unincorporated Lake County for recreational and open space usage by Lincolnshire residents.

Village of Gurnee

Shall the Village of Gurnee rezone or otherwise permit Six Flags Great America, Inc. and/or Prism Development Company, LLC to develop 134 acres of open space situated west of Interstate 94 and north of Washington Street to be operated for theme park uses including an 'entertainment village' and employee housing facilities?

Village of Hawthorn Woods

Should the Village of Hawthorn Woods approve the proposed development of a Super Regional Shopping Mall on the approximately 108-acre site at the southeast corner of the intersection of U.S. Route 12 (Rand Road) and Old McHenry Road?

Shall the burning of leaves be prohibited within the Village of Hawthorn Woods?

 

SCHOOL DISTRICTS

Millburn Community Consolidated School District Number 24

Shall the Board of Education of Millburn Community Consolidated School District Number 24, Lake County, Illinois, acquire land for, improve the site of, build and equip additions to and alter, repair and equip the Millburn Community Consolidated School Building and issue bonds of said School District to the amount of $9,900,000 for said purpose?

Upon the issuance of said bonds, an amount not exceeding 15% of the equalized assessed valuation of the taxable real property in said School District for the year 1998 will be outstanding in bonds if the proposed issuance of bonds is approved by the voters of said School District.

Community High School District Number 156

Shall the Board of Education of Community High School District Number 156, McHenry and Lake Counties, Illinois, be authorized to acquire a new high school site, build and equip school facilities thereon and improve said site, and alter, repair, renovate and build and equip additions to the West Campus facilities, and issue bonds of the District therefor to the sum of $46,000,000?

Antioch Community Consolidated School District Number 34

Shall the Board of Education of Antioch Community Consolidated School District Number 34, Lake County, Illinois, build and equip a new school building, build and equip an addition to and alter, repair and improve the Antioch Upper Grade School Building, and issue bonds of said School District to the amount of $11,850,000 for said purposes and for the purpose of purchasing and improving a site for said new school building?

Round Lake Community Unit School District Number 116

Shall the maximum annual tax rate for the educational fund of the Round Lake Area Schools, Community Unit School District Number 116, Lake County, Illinois, be increased and established at 3.64 percent on the equalized assessed value of taxable property instead of 3.24 percent the maximum rate otherwise applicable to the next taxes to be extended?

The total dollar amount of the most recently approved annual budget of the School District is $39,101,199 and the total dollar amount of that annual budget would be $40,132,189 if increased by the amount of additional tax which may be levied if the proposition is approved, a percentage increase of 2.64% of the total dollar amount of the most recently approved budget. If the current fund tax rate of 3.24% would be increased to a maximum rate of 3.64%, the percentage increase would be 12.35%. It is estimated that the approximate amount of taxes for educational purposes extendible under the maximum rate of 3.24% is $8,351,024.57, and that the approximate amount of taxes for educational purposes extendible under the proposed tax rate of 3.64% is $9,382,015.26.

Round Lake Community Unit School District Number 116

Shall the Board of Education of Community Unit School District Number 116, Lake County, Illinois, build and equip additions to its existing Round Lake Senior High School, Early Education Center and Indian Hill School, improve the sites of, and remodel, equip, alter and repair, the existing school buildings of said School District, and to pay the cost thereof, issue bonds of said School District to the amount of sixteen million dollars ($16,000,000)?

Bannockburn School District Number 106

Shall the maximum annual tax rate for educational purposes of Bannockburn School District Number 106, Lake County, Illinois, be increased and established at 2.21% upon all the taxable property of said School District at the value, as equalized or assessed by the Department of Revenue, instead of 1.96%, the maximum rate otherwise applicable to the next taxes to be extended for said purposes?

  1. The approximate amount of educational fund taxes extendible under the maximum rate now in force in said School District is the sum of $2,389,441.

  2. The approximate amount of educational fund taxes extendible under the proposed increased rate is the sum of $2,694,217.

  3. The annual rate at which educational fund taxes are currently levied is 1.96% and the percentage of increase between said rate and the maximum rate of 2.21%, if approved, is thirteen percent (13%).

  4. The total dollar amount of expenditures of the 1998-1999 budget of said School District, including bond and interest, is $2,448,640 and the total dollar amount of the expenditures of the 1998-1999 budget of said School District, including bond and interest, is estimated to be $2,753,416 if the proposition is approved, equaling a twelve percent (12%) increase.

Fox Lake Grade School District Number 114

Shall the Board of Education of Fox Lake Grade School District No. 114, Lake County, Illinois, build and equip an addition to and alter, repair and improve the Stanton School Building, and issue bonds of said School District to the amount of $12,300,000 for said purpose?

Woodland School District Number 50

Shall the maximum annual tax rate for educational purposes of Woodland Community Consolidated School District No. 50, Lake County, State of Illinois, be increased and established at 2.32 percent on the full, fair cash value of taxable property as equalized or assessed by the Department of Revenue instead of 1.87 percent, the maximum rate otherwise applicable to the next taxes to be extended for said purpose?

The approximate amount of educational taxes extendible under the maximum rate now in force in said School District is the sum of $16,726,968.

The approximate amount of educational taxes extendible under the proposed increased rate is the sum of $20,752,174.

The total amount of the most recently approved annual budget of the School District is $33,170,835.

The total amount of the budget increased by the amount of additional tax which may be levied if the proposition is approved is $37,196,041.

The percentage of increase in the total dollar amount of the most recently approved annual budget of the School District if such total dollar amount were increased by the amount of additional tax which may be levied if the proposition is approved is 12.1 percent.

The percentage of increase between the maximum rate at which the tax for educational purposes may be levied if the proposition is approved and the annual rate at which such tax currently is levied is 24.1 percent.

Grayslake Community Consolidated School District Number 46

Shall the Board of Education of Grayslake Community Consolidated School District Number 46, Lake County, Illinois, acquire land for school purposes, build and equip two school buildings, and issue bonds of said School District to the amount of $23,195,000 for said purposes?

Beach Park Community Consolidated School District No. 3

Shall the Board of Education of Beach Park Community Consolidated School District Number 3, Lake County, Illinois, acquire land for school purposes, build and equip a new school building, alter, repair and equip existing school buildings, and issue bonds of said School District to the amount of $14,600,000 for said purposes?

Township High School District Number 113

Shall the Board of Education of Township High School District Number 113, Lake County, Illinois, for its Highland Park and Deerfield High Schools, construct and equip additional instructional classrooms, replace and equip science labs, construct and equip computer labs, expand library/instructional materials center space, repair and replace infrastructure, improve the safety and efficiency of the physical plants, build and equip additions to such schools, remove, renovate and repair portions of such schools, and improve the sites for such schools and issue bonds in an amount not to exceed $70,000,000 for such purposes?

William Rainey Harper Community College District 512

Shall the Board of Trustees of William Rainey Harper Community College District No. 512, Counties of Cook, Kane, Lake and McHenry and State of Illinois, be authorized to build, equip, alter and repair community college buildings and building additions, including a science and technology center, and acquire and improve buildings and land used or useful for community college purposes and issue the bonds of said Community College District to the amount of $124,800,000 for said purposes?

 

PARK DISTRICTS

Grayslake Community Park District

Shall the Grayslake Community Park District, Lake County, Illinois, be authorized and empowered to levy and collect a tax of not to exceed .04% for the purpose of funding the Park District=s share of providing recreational programs for the handicapped as provided in Sections 5-8 and 8-10b of the "Park District Code"?

 

PUBLIC LIBRARY DISTRICTS

Zion-Benton Public Library

Shall the annual public library tax rate for The Zion-Benton Public Library District, Lake County, Illinois, be established at .22% of full, fair cash value instead of at .15%, the maximum rate otherwise applicable to the next taxes to be extended?

Ela Area Public Library District

Shall the bonds of "The Ela Area Public Library District, Lake County, Illinois," in the amount of Thirteen Million Nine Hundred Thousand Dollars ($13,900,000) be issued for the purpose of erecting a building to be used as a library, purchasing a site for erecting such a building, furnishing necessary equipment therefor, and acquiring library materials and electronic data storage and retrieval facilities in connection therewith?

Cook Memorial Library District

Shall the bonds of the Cook Memorial Public Library District, Lake County, Illinois, in the amount of $25,430,000 be issued for the purpose of erecting two new library buildings: one, in the downtown of the Village of Libertyville on or within 750 feet of the current library site containing not less than 70,000 square feet and a second in the Village of Vernon Hills in the vicinity of Aspen Drive and Route 60 containing not less than 15,000 square feet, furnishing necessary equipment therefor and acquiring library materials and electronic data storage and retrieval facilities in connection therewith?

 

FIRE PROTECTION DISTRICTS

Prospect Heights Fire Protection District

Shall the Prospect Heights Fire Protection District levy a special tax at a rate not to exceed .05% of the value of all taxable property within the district as equalized or assessed by the Department of Revenue for the purpose of providing funds to pay for the costs of emergency and rescue crews and equipment?

Fox River Grove Fire Protection District

Shall the Fox River Grove Fire Protection District levy a special tax at a rate not to exceed .015% of the value of all taxable property within the District as equalized or assessed by the Department of Revenue for the purpose of establishing a firefighters' pension fund?
