Request Records Electronically

You may order a certified copy of a birth, death, marriage or civil union record by fax or email using your credit card.

* Any individual who commits fraudulent use of a vital record is guilty of a Class 4 Felony, punishable by up to three years imprisonment.

Fees
The County Clerk fee to purchase a birth or marriage record is $10.00 for the first certified copy and $4.00 for each additional certified copy of the same record. The fee for a death record is $14.00 for the first copy and $8.00 for each additional copy. In addition, a $7.00 VitalChek handling fee per order will be charged to your credit card. The total cost for one certified birth or marriage record by UPS 2nd Day Air will be $33.50, or $37.50 for a certified death record. The total cost for one certified birth or marriage record by UPS Next Day Air will be $36.00, or $40.00 for a certified death record. Records will not be sent by U.S. mail.

Required Information
All requests must include:

  • Type of credit card, credit card number and expiration date
  • Address where the record is to be sent
  • The type of delivery method UPS Next Day Air or UPS 2nd Day Air
    Note: UPS cannot deliver to a post office box
  • Daytime phone number
  • Number of copies for each record requested
  • Photocopy of valid identification with signature
  • Requestor's signature

You must send your request to the County Clerk's office (Fax: 847 360-3608, Email: VitalRecords@lakecountyil.gov) and include specific information required for the document.

If you are requesting a death record, please include the following information:

  • Deceased's name
  • Date and place of death
  • The requestor's relationship to person on death record
  • Purpose or interest for requesting the record

If you are requesting a marriage record, please include the following information:

  • Names of both applicants on marriage license
  • Date and place of marriage

If you are requesting a civil union record, please include the following information:

  • Names of both ​applicants on civil union license
  • Date and place of civil union

If you are requesting a birth record, please include the following information:

  • Name on birth record
  • Date of birth
  • Father's name
  • Mother's maiden name
  • The requestor's relationship to person on birth record
  • Purpose or interest for requesting the record

In Illinois, you may request a copy of your own birth record if you are over 18, or parents may request a copy of their child's birth record. Other individuals requesting birth records must include, as part of the request, written authorization from the individual or parent (Authorization Form). This authorization MUST be notarized.

A photocopy of the requestor's valid identification with signature must accompany the request.

Request Form
The use of a request form is NOT REQUIRED. Requests will be accepted as long as the required information is included in the request. Alternatively, you may submit the required information using the third-party service VitalChek.

Credit Cards
VitalChek accepts Visa, Master Card, American Express or Discover cards.

Delivery
When requesting the record, please specify the type of delivery you would like. The fee for UPS Next Day Air is $19.00. UPS 2nd Day Air is $16.50. Please note, overnight delivery will not deliver to a post office box. For International rates, please contact the County Clerk's Office 847 377-2411.
